​Cremation Authorization

Educational Body Donation will require a completed Cremation Authorization form, and will make the final cremation arrangements based on this completed form.  All cremation services for Educational Body Donation will be completed by Cascade Cremation Center.  The cremated remains will generally be returned to a donor's family within 24 months from the time of donation.

Oregon Revised Statutes 97.130 defines who is authorized to control disposition of remains.
